 What do you need to do?

• Present your SmithRx card to your pharmacy beginning July 1 when you pick up prescriptions (see image below)

• If you have a 90-day prescription through the mail, call your doctor’s office and tell them your prescription carrier is now SmithRx. You do not need to be seen by your doctor to make this change. If you have any problems, contact SmithRx Member Support at 1-844-454-5201

• Here is a link to instructions on how to log into the member portal. Be sure to create your log-in and set up your account

• Be responsive if you get a call from SmithRx to discuss ways to save additional money on your prescriptions

• Open and read any mail you receive from SmithRx or LIBERTY. Your permanent ID cards will be mailed to you from LIBERTY as soon as they are available

• Below is a generic temporary ID card you can use beginning July 1, 2024, until your permanent individualized ID card comes in the mail:

There will be a 90-day grace period to have your prescriptions moved to SmithRx, but the sooner you take this step, the sooner you can start saving. Questions or concerns?
